MPLS traffic flow

I've worked with customers who had MPLS, but since it's managed by the ISP, I realized I don't really understand the traffic flow. In a hub-and-spoke setup, where the main office has higher bandwidth like 10 Mbps and the branch offices have something smaller like 4 Mbps, are all locations funneling internet traffic out of the main office's 10 Mbps connection, or are each of the 4 Mbps connections going out to the internet also? For instance, I'm wondering if two idiots at different branch offices streaming video would kill the main office's bandwidth. And along the same lines, if a remote office has a server that needs to be on its own public IP, does that traffic from the internet enter at the main office or the remote office?
